Our Care Management Mission
Across our unparalleled 20-year history as a local provider of quality care, our core mission has remained the same: to improve the health of the Medicaid population within the 34 Eastern North Carolina counties in which we are privileged to serve, by increasing the value of healthcare received within a medical home, and coordinating care through a collaborative network of primary-care physicians, care managers, hospitals, and public-health and social service agencies.
