Provider Tools
Our Community Care of North Carolina (CCNC) partners have assembled an extensive set of Provider Tools to assist primary-care providers (PCPs) and independent practices in creating more efficient patient-driven point-of-care. These decision-making aides, including algorithms based on current evidence and best-practice standards, are arranged both by medical condition and patient populations.
Note that CCNC routinely updates these resources, which also include tools specific to Medicaid itself.
