NC ROOTS Hub Medicaid Region 6
Access East's primary role in managing the ROOTS Hub initiative for Region 6 will be to help build strong networks of regional partners who can work together to improve health care, and access to health care services, in our underserved eastern North Carolina (ENC) rural communities. The work will involve:
- Making it easier for patients to receive primary care
- Helping local doctors and clinics, and service and community organizations, to collaborate more easily and efficiently
- Helping to boost the regional health care workforce to meet ENC’s unique needs
- Using current health care data and digital tools to improve local care
For more than 30 years, Access East has coordinated comprehensive, patient-centered care and community support for many of our ENC neighbors who need it most. We are honored to be included in this critically important state initiative to improve the health of our rural communities.
NC Medicaid Region 6 includes 27 of North Carolina's 100 counties, and some of the most rural areas of the state.
