What is the Rural Health Transformation Program?

The N.C. Department of Health and Human Services (NCDHHS) designed its Rural Health Transformation Program, or RHTP, to improve health care for rural residents within their own communities.

The RHTP has now launched NC ROOTS (Rural Organizations Orchestrating Transformation for Sustainability), to lead that effort in each of the state's Medicaid regions. RHTP is a five-year, statewide initiative to build collaborative, locally managed and regionally unique networks of medical, support and behavioral health professionals and services.

The state's six ROOTS Hubs will connect providers, service and faith organizations, community partners and local leaders to collaborate on strengthening, and improving access to, quality health care and health-related social services for underserved rural residents.

An estimated 3 million North Carolinians live in rural areas, the second-largest rural population in the U.S., with eastern N.C. being home to one of the greatest concentrations of rural residents in the state.

NCDHHS has chosen Access East to lead the ROOTS Hub for Medicaid Region 6.
