Community Partner Rural Health Survey
The NC Rural Health Transformation Program’s ECU Health Region 6 Team invites you and your eastern North Carolina neighbors in Medicaid Region 6 to take part in an important survey on the strengths and weaknesses in health care within your own community.
The survey closes Aug. 15.
Your answers will help shape health programs over the next several years through the NC ROOTS Hub.
Health Care Professional Rural Health Survey
The NC Rural Health Transformation Program’s ECU Health Region 6 Team invites medical practitioners, administrators and other stakeholders who work within or directly with regional health care to take part in an anonymous, 15-minute needs-assessment survey to help shed greater light on those health services, and on the health care workforce and care coordination, and any gaps in any of those areas.
The survey closes Aug. 15.
The findings will inform how the NC ROOTS Hub prioritizes funding and programs over the next several years.
